Tribute Donation Notification (Optional)

If you are making this donation in honour of or in memory of someone, you may provide the name and contact information (email or address) of a family member, friend, or other individual you would like us to notify in the notes or email boutique@lenichoir.org. 

We will use this information only to send an acknowledgement that a donation has been made in honour of or in memory of the individual named. We will not add the recipient to our mailing lists or use their information for marketing purposes without their separate consent.

By providing another person’s personal information, you confirm that you have their permission to share it with us for this purpose.

If no contact information for the honouree is provided, we will contact the donor directly and provide them with acknowledgement to share with the recipient.

Tax receipts will be emailed to you.
